Who we are

The Cognition Self-Assessment Rating Scale (C-SARS) is offered by the CURESZ Foundation, 675 Deis Drive, Suite N, Box 192, Fairfield, Ohio 45014. Questions about this notice or your information can be sent to [email protected].

What the C-SARS is

The C-SARS is a free, voluntary self-assessment. It is not a diagnosis, does not replace an evaluation by a qualified professional, and does not create a patient–provider relationship with CURESZ. It is intended for adults 18 and older.

Information we collect

We collect only your answers to the 12 C-SARS questions. We do not ask for your name or email address. The form calculates a frequency score and a severity score from your answers, and records the date and time of your submission. We do not store your IP address. We do not save forms that are started but not submitted. All of this information comes directly from you.

How we use it

We use your answers only to calculate your scores, show you your results and let you download them as a PDF. We do not email your results, and we do not use your responses for research or any other purpose.

Who can see it and who we share it with

While your entry is stored, it can be viewed only by authorized CURESZ staff and by Geek Genius, the firm that builds and maintains our website. We use the following service providers, which handle information only to provide their services to us:

  • Liquid Web, our website hosting provider, which stores the website and its nightly backups.
  • Google Drive, which CURESZ uses to receive a quarterly backup copy of the website.

Our website uses Google Analytics and Google Search Console to understand general site traffic. These tools may record that you visited the C-SARS pages, but Google Analytics is not connected to the C-SARS form, and your answers and scores are not sent to Google.

We do not sell your information, use your answers for advertising, or use them to decide whom to ask for donations. We will not otherwise share your responses unless you ask us to or we are required to by law.

How long we keep it

The link to download your results PDF works for 20 minutes after you finish, so please download your results right away. Your C-SARS entry and the PDF are deleted automatically from our website one day after you submit. Nightly website backups are kept for 5 days, so a copy may remain in a backup for up to about 6 days. If a quarterly backup happens to be made while your entry is still stored, a copy may remain in that backup, which CURESZ keeps until it is replaced by the next quarterly backup, up to about three months. The PDF you download stays on your own device until you delete it.

Your choices and rights

Taking the C-SARS is voluntary, and you may stop at any time before submitting. You may ask us to send you a copy of your information, correct it, delete it, or stop using it, and to tell you which service providers have received it. Because entries are deleted within one day, we usually will no longer have your responses by the time we receive a request; if a copy remains in a backup, we will tell you when that backup will be deleted. Send your request to [email protected]. We will respond within 45 days. If we decline your request, you may appeal by replying to our decision, and we will respond to your appeal within 45 days.

Security

We protect the website with encrypted (HTTPS) connections, a firewall, malware scanning and security monitoring, and we limit who can view responses. No website can guarantee complete security.

Changes to this notice

If we change how we handle C-SARS information, we will update this notice and its effective date before the change takes effect. We will ask for your consent again before using your information in a materially different way.
